Our Patient and Family Support service provides emotional, spiritual and practical support to people diagnosed with a life
- limiting illness and those around them. Our team of palliative care social workers, social work assistants, chaplains and volunteers help people with expressing their feelings, making decisions, facilitating difficult discussions and creating memory making resources.


Our Bereavement Service supports those who have experienced loss to find a way forward that includes remembering the person who died in the way that they want.

Our trained volunteers offer spiritual and pastoral care through individual support sessions, group support sessions and remembrance ceremonies.


We are now looking for a Head of Patient and Family Support Services to join us on a full-time, permanent basis.

The Role


As the Head of Patient and Family Support Services, you will manage and lead the delivery of Social Work, Spiritual Care and Bereavement Services.


Acting as the safeguarding lead for the organisation, you will oversee all members of the PaFs team, ensuring the appropriate training, systems and safeguarding procedures are in place to ensure a safe, effective and high-quality service is delivered.


Managing the volunteers within the Bereavement Service, you'll ensure the delivery of an inclusive and diverse service that is focussed on offering spiritual care to encourage openness, understanding and peace for the patient or visitor.


Monitoring and reviewing the PaFs risk management systems, you'll seek areas of improvement and ensure that safeguarding incidents are addressed and investigated.


Additionally, you will:


  • Ensure compliance with Care Quality Commission standards and organisational policies
  • Deliver the Social Work provision to the Inpatient Unit
  • Contribute to the development of the Clinical Strategy
  • Support fundraising initiatives
  • Manage the PaFs financial budget
About You

To be considered as the Head of Patient and Family Support Services, you will need:

  • Experience of leading service delivery
  • Management experience
  • Experience of leading and managing change
  • Experience of delivering quality improvement projects
  • The ability to lead and motivate a diverse team
  • Knowledge of the Care Quality Commission key lines of enquiry and governance
  • Familiarity with the Safeguarding Care Act 2014 and Mental Capacity Act 200
  • Excellent communication skills
  • To be registered with Social Work England (SWE)
  • A social work DipSW and/or degree or an equivalent qualification
