  • Salary - £36,100 - £43,470 per annum (pro rata for part time)

  • The role of the Community Senior Staff Nurse/Paramedic is to support the delivery of planned and unplanned care across Rennie Grove and Peace Hospice to provide high quality specialist palliative nursing care, skills, advice and support to patients, carers and those important to the patient.

    You will work across Rennie Grove hospice sites, but be based at either Gillian King House Chalfont St Giles, Rennie House Tring or Grove House St Albans, so a car and driving licence will be essential.

    Main duties of the job

    Key Activities will include:

  • Participating in the development of the Rennie Grove Peace Hospice planned and unplanned community Hospice @ Home model of care.
  • To be responsible for those patients allocated on a daily basis
  • To ensure that excellent standards of support and clinical practice and care are achieved.
  • Deliver specialist palliative nursing care to patients requiring planned and unplanned care. This will include a multi-disciplinary approach for our higher acuity patients with daily virtual and in person reviews in their own homes.
  • Improve patient outcomes and experience by providing physical and emotional support to patients, coordinating their care services.
  • Act as a Rennie Grove Peace Hospice Care ambassador within the community, actively demonstrating our values and positively promoting the specialist care we provide.
  • Work across Rennie Grove Peace Hospice Care geographical area (as required) to support safe and effective service delivery.

    The purpose of the role is to provide high quality specialist palliative nursing care, skills, adviceand support to patients, carers and those important to the patient.     Actively participate in the development of the organisations planned and unplanned communityHospice at Home model of care. To work as part of the multidisciplinary team to provide a high standard of individualised patient /family care and support.

    To be responsible for those patients allocated on a daily basis.

    To ensure that excellent standards of support and clinical practice and care are achieved.

    Deliver specialist palliative nursing care to patients requiring planned and unplanned care. This willinclude a multi-disciplinary approach for our higher acuity patients with daily virtual and in personreviews in their own homes.

    To work with wider Hospice team as well as system partners to support delivery of care and enablepatients to remain in their preferred place of care

    Improve patient outcomes and experience by providing physical and emotional support to patients,coordinating their care services. Participate in activities such as audits and case reviews in order to improve the Hospice at Homeservice

    Act as a Rennie Grove Peace Hospice Care ambassador within the community, activelydemonstrating our values and positively promoting the specialist care we provide.

    Work across the Rennie Grove Peace Hospice Care geographical area (as required) to support safeand effective service delivery.

    Person Specification

    Qualifications

    Essential

  • First level Registered Nurse or Paramedic.
  • Willing to or working towards of have completed further study in Palliative Care or relevant area.
  • Experience

    Essential

  • Proven palliative care experience or related speciality.
  • Proven experience of effective team working.
  • IT literate including a good knowledge of Excel, Word, a clinical IT system.
  • Desirable

  • Evidence of extended roles within their practice.
  • Experience working with volunteers.
  • Skills and Knowledge

    Essential

  • Excellent written and verbal communication skills.
  • Good organisational skills.
  • Able to prioritise clinical duties.
  • Ability to work autonomously and as an effective member of the team.
  • Desirable

  • Advanced communication skills training or commitment to achieve this.
