    Job Title: Nurse Practitioner for Urgent Community Response/Virtual Ward Team

    Company: Healthcare Provider in Stockport

    We have an exciting opportunity for a Nurse Practitioner to join our team.

    We are looking for a dynamic and enthusiastic practitioner to support the delivery of Urgent Community Response and Virtual Ward mandates to improve out-of-hospital care and patient management at home.

    The team provides urgent care at patients' homes, helping to prevent hospital admissions and promoting independent living. Join us as we expand the Virtual Ward/Hospital at Home service.

    Main Duties:

    • Assess, plan, implement, and review patients in their own home
    • Monitor and enhance standards of care

    Person Specification:

    Education and Training

    Essential

    • Registered Nurse with relevant qualifications
    • Prescribing qualification or working towards it
    • Teaching and mentoring qualifications

    Desirable

    • Urgent care or ED experience
    • Specialist qualifications

      • Understanding of healthcare legislation
