The Hospital at Home Teams in Fife offer a valuable service for the elderly population, mainly those over 65 years old. This service aims to prevent hospital admissions or enable patients to return home early after being hospitalized. It allows individuals to receive acute medical care in the comfort of their own homes, promoting a faster recovery without the need for hospitalization.
After a successful trial period integrating Hospital at Home and acute services, we are thrilled to recruit 2 Nurse Practitioners to join our established Hospital at Home team. These individuals will play a crucial role in the core Hospital at Home Service, providing support at Victoria Hospital. This includes working in the acute hospital, assisting with discharges, and conducting clinical assessments for patients transitioning to the Hospital at Home Service. Additionally, they will rotate through different Hospital at Home teams across Fife.
The Nurse Practitioners selected must deliver high-quality, safe, and effective acute medical care predominantly in the acute setting, with occasional support for the broader Hospital at Home service in the community. They should be capable of working independently in this dynamic healthcare environment and hold a degree-level qualification in clinical decision-making and non-medical prescribing. Furthermore, skills such as venepuncture, ECG, IV additives, and cannulation are essential for this role. Strong communication, confidence, assertiveness, and time management skills are vital.
The role also requires the ability to travel promptly within the area to support the Hospital at Home service, respond to deteriorating patients, and conduct routine clinical interventions in the community.
