We have created a new and exciting role in our team for an enthusiastic and proactive Assistant Practitioner within Diabetes nursing.


You should have significant prior experience of working with patients who have a diagnosis of diabetes in the acute/community setting.


The post-holder will work as an Assistant Practitioner (AP) within the Diabetes Specialist Nursing Team, to provide a high standard of support, care and education to patients and their carers' in secondary care.

To undertake a range of delegated tasks and delegated authority, reporting to a DSN


It is anticipated that this role will continue to develop through the acquisition of further skills, knowledge and competencies to be determined within the clinical teams, with focus on client need.

Planning and Organisational Duties

  • To adhere at all times in accordance with all Trust policies, procedures and protocols specific to the service.
  • To maintain accurate patient care records in accordance with the Trust Guidelines for Records and Record Keeping.
  • To be involved in aspects of progressive information technology, i.e. documentation of patients care on HIS and communicating with GP's and other HCP in timely manner. Participating in audit and data input.
  • To contribute to the administration duties required by the service. For example, telephone enquiries, filing patient's notes.
  • To ensure information is accurate and up to date for future reference and complies with Data Protection Act.
Responsibility for Health & Safety

  • Compliance with the Health & Safety at Work Act the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
Responsibility for Teaching

  • The post holder will be responsible for the teaching of patients in all aspects of diabetes management following assessment of the patient by the DSN.
