This is an exciting opportunity for a Band 6 Registered Mental Nurse to join our Adult Community Mental Health Team based in Sale which provides a service to people living across the borough of Trafford.
The North Community Mental Health Team in Trafford is an integrated health and social care service and strives to provide quality recovery-based support to people with severe and enduring mental health concerns.
Under the line management and clinical supervision from within the CMHT, the post holder will maintain and co-ordinate timely and appropriate interventions for a caseload of service users living in the community within the framework of CPA, in line with relevant clinical pathways as outlined within the Adults CMHT operational procedure.
This includes respecting the individuality, values, culture and religious diversity of clients and colleagues and thus contributing to the provision of a service sensitive to these needs.
Under the line management and clinical supervision from within the CMHT, the post holder will maintain and co-ordinate timely and appropriate interventions through holistic assessment for a caseload of service users of any age living in the community within the framework of CPA, and in line with the acute care pathway as outlined within the CMHT operational procedure.
