This role will be hosted by South East Leeds GP Group, and be based in the Middleton & Hunslet Primary Care Network (PCN) which covers the following 4 GP Practices located in LS10:

  • Lingwell Croft Surgery
  • Southbank Surgery
  • Arthington Medical Practice
  • Church Street Surgery

We are looking to recruit a Health and Wellbeing coach to join our existing team to support and empower our patients to set goals and manage their own health and wellbeing.


Main duties of the job


The post holder will be a key member of the multi-disciplinary team, helping patients and their carers to increase their participation in, and understanding of, their health condition, by supporting and empowering them to make the most appropriate and informed decisions to manage their condition focusing on obesity and diabetes.

Our PCN pharmacists are currently developing a Diabetes Support Team (DiaST) and we are looking to the newly appointed Health and Wellbeing Coach to play an important role in this project by engaging patients and using health coaching techniques to support and empower them to take an active role in their health.

You will work with patients of low to medium complexity that may have one or more long term conditions.


We are looking for someone who is passionate about the role and the opportunity it must make a difference to the health and wellbeing of our patients.

This is a new role, so we are looking for someone who is flexible, self-motivated, willing to embrace change and proactive in continuously improving service delivery.

Job responsibilities


In addition to working within the immediate team, you may also work with our colleagues and partners from the statutory, voluntary and community sectors including our Local Care Partnership (LCP).


As part of this role, you will be expected to work across various sites and settings within the community e.g., you could work in care homes, within a practice, or from a community venue within the PCN.

You may on some occasions be required to work evenings and weekends to meet the needs of the patients.

Key ResponsibilitiesClinical


To work as part of a multi-disciplinary team in a patient facing role to assess and work with patients using expert knowledge.

Proactively identify patients with long term conditions from practice clinical systems.

Maintain own caseload working on a 1:1 and group basis

To take an approach that is non-judgmental, based on strong communication and negotiation skills, while considering the whole person when addressing existing issues

Working with others in the PCN practices to undertake NHS health checks in line with protocols.


To coach, motivate and empower patients through multiple sessions to identify their needs set goals, and support them to implement their personalised health and care plan, signposting to additional services as required.

Support patients through the process and provide interventions, tools and techniques to enable patients to achieve their personal goals.


Responsibility for providing support to patients who will benefit from proactive health management and care, including being the single point of contact for a person to simplify access and coordination of services.

Provide personalised support to individuals, their families and carers to ensure that they are active participants in their own healthcare, and to empower them to take more control in manging their own health and well-being, to live independently and to improve their health outcomes through:
Providing interventions such as self-management, education and peer support, with those living with Diabetes

Supporting people to establish, attain and build on goals set by the person and based on what is important to them

Working with the social prescribing services to connect them to community-based activities which support their health and well-being

To be aware of limitations and, as required, be responsible for the referral of patients who require the intervention of other healthcare professionals

To work with patients and to understand their level of knowledge, skills and confidence when engaging with their health and well-being

To work with and provide support to local community groups and work with other health, social care and voluntary sector providers to support patients health and well-being holi
