Salary Band:
Trinity Grade C (£23,836 - £25,187 pa)

Contract Term: 1-year fixed term (Maternity Cover)**
Working Hours: 37.5 hrs per week worked in 12 hour shifts from 8am to 8pm. Inclusive of weekends and public holidays .

An exciting opportunity has arisen to join our WEOLCCS team as a Care Co-ordinator. Would you enjoy a role working within a team that makes a real difference?

This role plays a vital part in the goal of the end-of-life care system in Wandsworth, supporting patients to achieve their preferred place of care and death, to avoid unplanned and inappropriate hospital admissions and to support rapid discharges from hospital.

The nature of the role will require you to use your initiative and will involve interactions with distressed and anxious patients, their carers and families.
