    Acqhired is recruiting on behalf of a leading domiciliary care provider based in Sutton Coldfield, committed to delivering high-quality care services to our clients in the comfort of their own homes

    Role Overview:
    They are seeking a passionate and dedicated Care Coordinator to join their team.

    The Care Coordinator will be responsible for managing and coordinating care services for their clients, ensuring that all care plans are tailored to meet individual needs and preferences.

    The successful candidate will work closely with their care staff, clients, and their families to ensure the delivery of person-centred care that promotes independence, dignity, and well-being.


    Key Responsibilities:
    Coordinate and schedule care visits, ensuring that all allocated hours are effectively managed and utilised.
    Conduct client assessments and develop personalised care plans in collaboration with clients, their families, and care staff.

    Monitor and review care plans regularly to ensure they meet the changing needs of clients and comply with regulatory standards.

    Conduct regular supervisions and appraisals for care staff, providing constructive feedback and identifying areas for development.
    Liaise with healthcare professionals, social services, and other external agencies to coordinate care services and support multi-disciplinary working.
    Maintain accurate and up-to-date records of client assessments, care plans, and staff training and development.

    Respond promptly to any concerns or complaints raised by clients or their families, taking appropriate action to resolve issues and improve service delivery.


    Key Requirements:
    Previous experience working within a domiciliary care setting or similar role is essential.

    Strong organisational and administrative skills, with the ability to manage multiple tasks and priorities effectively.12 months experience within a Care Coordinator role is essentialExcellent communication and interpersonal skills, with the ability to build positive relationships with clients, their families, and care staff.

    Knowledge of relevant legislation, regulations, and best practice guidelines within the care sector.
