Description

We are excited to share an opportunity to join our team of elderly care consultants here at East Cheshire NHS Trust.

A unique opportunity has arisen for two Consultant Geriatrician's to support our new and developing Virtual Ward frailty service as well as supporting acute frailty services in line with the Trusts vision.

The Trust is integrated with both Acute and Community Care and is committed to providing coordinated, joined up care for older adults living with frailty across the community-hospital interface.

The Trust has recently re-aligned its Crisis Response services with clear opportunities to strengthen links across the interface between the two services.

Virtual community ward models are also in early development with the potential to grow these services further with appointment to this role.

There is an emphasis on an integrated, whole system approach to older people's care in East Cheshire as part of the NHS long term plan.

Our elderly care services have recently been developed to bring our specialty teams together and improve the care of patients in our local community.

Our services are currently evolving, and by joining us at this exciting time it will give you the unique opportunity to lead the development of new services across hospital and community settings.

Based in Macclesfield District General hospital, the Care of the Elderly Department is expanding, dynamic and forward thinking.

Over the last few years, there have been several recent appointments and there are now 3 consultants working in the team with an interest in transforming older people's care at the trust.

The department is highly rated by patients and their relatives for providing excellent care.

The Care of the Elderly Department includes 2 Elderly Care medical wards, Ortho Geriatric provision and a community intermediate care ward (Ward 2, Ward 9, Aston)

Ward 9 has recently been refurbished and redecorated to dementia-friendly standards and has a newly appointed activity co-ordinatator. The ward has a fantastic team of nursing staff with specialist skills in dementia care.

If you have lived or worked abroad for 6 months or more in the last 5 years you will need to obtain an overseas police clearance.
