We are looking for 4 x band 7 Clinical Team Leaders to work as In Reach Discharge Coordinators facilitators.

Each of the coordinators will be aligned to one of the following teams Service Base and team East and Mid HTT 1x band 7 Gatton Place SW HTT 1 x band 7 Farnham Road Hospital NWSHTT 1 x band 7 Braemdean NEH SH HTT 1 x 7 Theta The InReach/Discharge Pathway Co-ordinator is an integral function of the Home Treatment team.

The successful applicant will work across the acute service line to identify and support patients who may be suitable for support in the home environment, and work towards supporting the capacity and flow of patients.

To oversee the coordination of discharges you will keep an accurate log of all admissions through your aligned home treatment team and understand their progress and readiness for Criteria for Discharge being met.

There will be travelling with this role to see people in various inpatient units. You will actively check and challenge treatment plans to ensure that treatment plans are addressing purpose of admission.

You will work closely with identified carers and significant others to prepare for timely discharges You will work closely and communicate effectively with your aligned CRHTT to actively follow up those being discharged from hospital.


To Identify and support early patient discharges to the Home Treatment clinical team in order to facilitate early discharge / leave for adult inpatients services within the Surrey and Borders NHS Foundation Trust.

To provide a gatekeeping service report for the HTT including estimated time of leave / or early discharge data. Support the coordination of admission, discharge process within both the community and inpatient services. To support facilitated HTT discharges from the acute wards for HTT management.


To be an active member of the MDT supporting the discharge process and facilitating all discharges in an effective and timely manner including attending ward rounds, for potentially identified patients.

Daily liaison with wards to understand plans for any patient movement e.g. leaves / discharge. Escalate and issues with early discharges with the Clinical Services Manager.


Provide early warnings of potential risk / issues taking into consideration potential solutions and taking corrective action as appropriate and / or escalating as required regarding bed flow.

Ensure the patients care responsibility lies with SABP.

To liaise with the Inpatient / HTT Service Manager, BedFlow, Stakeholders on a daily basis to inform of the current / potential movement within the organisation.

The information maybe both complex and contentious requiring persuasion and negotiation in order achieve optimum utilisation of beds.

The purpose of this post is to identify and facilitate the early discharge and leave for patients who will be supported in the community by the Home Treatment Team.

The post holder will be productive in ascertaining an accurate Locality position regarding the overall bed management status which will then be utilised by the patient flow hub for managing beds.

The post holder will assist the HTT aligned services ensuring delayed discharges are progressed through the proper channels and resolutions are met in a timely manner.

They will be responsible for providing a fully comprehensive and updated gatekeeping service report to the HTT.
