Description

The Haematology Ward at the Churchill Hospital is seeking a proactive and hands-on Discharge Coordinator to join our dynamic team.

The post holder will facilitate effective, efficient and timely discharge of patients from hospital.

Particular emphasis is placed upon complex multi-disciplinary assessments and case conferences for patients with complex health and social care, palliative care or rehabilitation needs.


  • To work as a member of the ward team and work with the wider multidisciplinary team to facilitate patient discharges in a timely manner.
  • Assist in the process of assessing and making appropriate and timely discharge plans for patients ensuring that all relevant information and assessments are up to date.
  • Effectively communicate information to patients, relatives, carers and all members of the multidisciplinary team.
  • Encourage the patient's family and friends to be actively involved in the discharge process.
  • To liaise with other providers in ensuring that discharge plans for patients are proactively managed and supported throughout the ongoing discharge planning process.
  • To be a point of contact on the ward providing advice and support around patient discharges, liaising with clinical teams as required.
  • To record up to date patient information including social history, onto relevant EPR/IT systems.
  • Work with the nursing team to ensure discharges are prioritised on a day to day basis.
  • Working with OT and Physiotherapy teams to plan, implement and evaluate programmes of therapeutic activity for patients.
  • To actively coordinate discharge plans of patients to be transferred, discharged or moved to the Discharge Lounge (as appropriate).
  • Make patient transport bookings as required for patients being discharged to minimise delays.
  • Ensure TTOs are ordered and ready to minimise delays to discharge.

Specific responsibilities will include:


  • Participate in multidisciplinary team meetings and actively contribute to the planning of patient discharges.
  • Develop and maintain effective working relationships with all members of the multidisciplinary team to ensure collaborative and holistic working for the benefit of patient care.
  • Share up to date discharge information with relevant staff, as well as patients and their families.
  • Implement and evaluate patient rehabilitation plans as suggested by relevant registered health care professionals.
  • Assess patients' mobility and issue appropriate equipment under the guidance of registered therapists, providing advice to patients/carers on safe use and care of equipment on discharge.
