    Job summary

    An exciting opportunity has arisen for a Senior Occupational Therapist to work within our community beds team.

    The team provide a community bed-based rehabilitation service across three community care home settings in the Nottingham City locality, often patients are admitted to the community beds directly from hospital but some are admitted directly from their own homes after experiencing a crisis that does not require acute secondary care.

    The team are based within the care homes and complete assessments, rehabilitation and discharge planning in a timely manner to support patients back into the community.

    The applicant will have a good knowledge of community services and an understanding of health conditions, especially those associated with ageing.

    You will work closely with Nurses, other Occupational Therapists, Community Care Officers, Assistant Practitioners, Rehabilitation Support Workers and Physiotherapists as part of a Multidisciplinary team and have developed strong leadership skills in supporting a team.

    This post may require shift working between 8am-4pm, 12-8pm Monday to Friday and 10am-6pm at a weekend and bank holidays.

    Full driving licence and the ability to travel between locations, so access to own vehicle is essential on a daily basis for work purposes

    Main duties of the job

    The candidate will be responsible for Occupational Therapy assessment and treatment for a defined clinical caseload appropriate to the service area. You will work as a senior Occupational Therapist, supporting junior members of staff along with facilitating student placements.

    It will be your responsibility to support clients to improve or maintain their level of function and quality of life and plan future care in an appropriate setting in partnership with multi-disciplinary team (MDT) colleagues, including liaising, supporting and referring on to health services, voluntary services and public sector agencies.

    The successful candidate will have the requirement to work as a key worker and be responsible, along with the wider MDT in planning a safe discharge from the service, including liaising with the client, families and other agencies to make decision on the future needs of the client.

    As a senior Occupational therapist, you will carry out training to other members of the teams and support junior occupational therapist and other members of the MDT such as assistant practitioners.

    There will be a responsibility for you to carry out training to other members of staff and participate actively in the services training strategy, including in service training organisational specific and multi-agency training, you will also be able to lead group sessions as we needed.

    Job Purpose

  • To work as an autonomous practitioner, you will undertake comprehensive assessments to identify the appropriate service for the clients referred.
  • To contribute to a high quality service by carrying out assessment, goal setting, implementing treatment programmes and evaluation of adult clients who may have complex and/ or chronic presentations including musculoskeletal, sensory impairment, medical and mental health needs, with support and supervision from senior physiotherapists.
  • To be responsible for continuing the ongoing development of high standards of clinical practice with evidence-based focus.
  • To support clients to improve or maintain their level of function and quality of life and plan future care in an appropriate setting in partnership with multi-disciplinary team (MDT) colleagues, including liaising, supporting and referring on to health services, voluntary services and public sector agencies.
  • To participate actively in the services training strategy, including in-service training, organisation-specific and multi-agency training.
  • To be responsible for planning a safe discharge from the service, including completing access and discharge visits.

  • To accept clinical responsibility for a designated caseload of clients, and to organise this effectively with regard to clinical priorities and use of time and ability to be flexible and respond at short notice to the needs of the service.
  • To provide highly specialist advice, education and mentorship to the other staff and colleagues within the Reablement team and other multi-professional teams around current best practice and evidence based practice regarding the clinical management of clients. To drive quality, clinical service improvement and clinical excellence.
  • To be responsible for own continuing professional development keeping abreast of new trends and developments. Attend relevant conferences and training courses, supporting junior staff to do the same. Incorporate new developments into clinical practice.
  • Working within a legal framework, gain informed consent to treatment and have the ability to identify through assessment, clients who lack the capacity to consent.
  • To participate in the supervision framework as a supervisee and a supervisor, including the appraisal process. To be able to set own objectives and evaluate own learning and that of more junior staff.
  • To be a clinical educator for undergraduate students on University placements, supervising, educating and measuring their performance against the standards set by the degree level qualification.
  • To demonstrate a sound understanding of Clinical Governance and Risk Management and apply to a work situation.
  • To conduct and report on clinical risk assessments as and when appropriate.

  • BSc/Diploma in Occupational Therapy

  • Experience

  • Relevant undergraduate/placement experience working with adults including Health Care of the Elderly and Older peoples mental health / Integrated Medicine, Orthopaedics, Neurology and Musculoskeletal
  • Understanding of multi-disciplinary and/or multi-agency working in a community setting
