Key Responsibilities:
    • The Practice Nurse will undertake a range of responsibilities including:
    • Conducting thorough health assessments and screenings to identify patient needs.
    • Administering vaccinations and immunizations following established protocols.
    • Supporting the management of chronic diseases such as diabetes and asthma through the implementation of comprehensive care plans.
    • Providing expert wound care management and assisting with minor surgical procedures as required.
    • Offering family planning services and providing reproductive health counseling to patients.
    • Delivering health promotion initiatives and educational sessions to patients and their families.
    • Collaborating with General Practitioners and other healthcare professionals to develop and implement individualized care plans.
    • Maintaining accurate and up-to-date patient records and participating in practice audits to ensure quality standards are upheld.
    Requirements:
    • The ideal candidate will possess the following qualifications and attributes:
    • Registered Nurse qualification with current NMC registration.
    • Proven experience working as a Practice Nurse within a primary care setting.
    • Competency in conducting health assessments and implementing appropriate nursing interventions.
    • Proficiency in administering vaccinations, wound care management, and minor surgical assistance.
    • Excellent communication skills and the ability to build rapport with patients and colleagues.
    • Strong team player with a collaborative approach to healthcare delivery.
    • Commitment to ongoing professional development and staying updated on best practices in nursing.
