    Job summary

    A vacancy has arisen for a Band 2 HCAto work with our Twilight service.

    Working two 5 hour shifts per week youwould be working with a qualified nurse to help deliver care to clients intheir homes in the evenings.

    Main duties of the job

    Experience of working as a carer isessential and community experience is desirable.

    The service works from 7pm til midnight7 days per week.

    Access to a car and ability to driveare required for this role.

    Job responsibilities

    2. Job Purpose

    We are looking for a Health Care Assistant to join our highly skilled and caring Twilight team working 2 x 5 hour evening shifts ( in the week and at weekends) on a 4 week set rolling rota. You will be providing high quality, evidence based care to patients in their own home.

    As a highly motivated individual, you will be used to working in a place that is constantly and rapidly changing. With lots of empathy for people, youll find the challenges of community nursing enjoyable. Treating all patients with respect and courtesy, maintaining dignity and respecting individual needs and differences, both physical and cultural.

The role will involve working closely with qualified staff to deliver the highest standard of care to patients throughout their care in the community setting.

    A valid UK driving licence plus the use of own transport for business purposes is essential.

    3. Dimensions

    This role supports the District Nursing Team in two localities across the Kingston borough

    in the delivery of integrated, person centred, responsive, co-ordinated and high quality

    care.

    4. Result Areas

    Clinical

    Carry out a range of duties under the direction of a registered community nurse in accordance with policies, procedures and guidelines including:

    Urinalysis

    Simple wound care

    Blood glucose monitoring

    Temperature and pulse recording

    Blood pressure monitoring

    Setting up of PEG/RIG feed

    Health promotion

    End of life communication

    Health education

    Communication and working relationships

    To build a professional rapport with patients and their families, communicate with them in a way that respects their views, autonomy and culture.

    To communicate effectively with all professionals concerned with the patients care.

    Ensure patient confidentiality is maintained at all times.

    Supervision and Appraisal - Review and reflect on your own practice through regular participation in professional supervision and appraisal.

    Service delivery - To ensure that clinical bags are well maintained and necessary equipment is prepared at the beginning of the shift

    Undertake clerical duties as required Using Carenotes patient record system to admit and discharge patients and to maintain diary sheets for each shift

    Professional development - To undertake relevant activities to meet the training objectives identified with your supervisor.

    Undertake mandatory training.

    To keep a record or your training and clinical development activities.

    Clinical governance - Participate in clinical governance and quality improvement projects as allocated by your manager.

    Personal management - To exercise good personal time management, punctuality and consistent reliable attendance.

    Qualifications

    Essential

  • Willingness to undertake further training and expand role within the District Nursing Team.
  • Experience

    Essential

  • Providing care for older people in the community or hospital setting.
  • Other factors

    Essential

  • Full valid driving licence and access to a car to use regularly for business purposes is essential.
  • Able to work between 7pm and midnight (6pm at weekends) including weekends and bank holidays as required.
  • Knowledge

    Essential

  • Knowledge of Primary Care and the role of Community Nursing.
  • Knowledge of Integrated Care Teams.
  • Knowledge of the role of the voluntary sector in the community.
  • Desirable

  • Experience of working within a Community Nursing Team
