This is an exciting opportunity to join our Planned Community Nursing Teams, based in the West Locality.


The post holders will be expected to work multi professionally, support other team member's roles and contribute to the future development of the service.


The post holder will work as part of the Community Team and will be accountable for the management and delivery of care for a delegated caseload.

Working with other members of the community nursing team to provide care to and promote health/well being and independence for patients on the team caseload, working closely with other partners/stakeholders including GP's, Early Intervention and Advance clinical practitioners in the prevention of inappropriate
hospital admissions and facilitation of hospital discharges in line with service objectives


As a senior nurse you will be able to demonstrate good leadership skills, the ability to manage change in clinical practice and processes.

You will also be able to demonstrate the ability to independently make sound clinical and leadership judgement and regularly supervise the workload of junior colleagues.

We are looking for people who are innovative, forward thinking, flexible and have strong leadership qualities.

Priority will be given to applicants who possess the District Nurse qualification or who are currently working towards this.

Service Hours


The service currently operates between the hours of 8am to 6pm over 7 days a week but these hours will be extended in the coming months to 8am to 8pm.

The working pattern will include weekend and bank holiday working.

This post requires a full driving licence and car available for work.
