  • Main purpose of job: Manage a team of workers who support adultsbeing discharged from hospital or living at home with no social care package, and work towards preventing readmission.

    What will you be doing?

  • Work in partnership with voluntary and statutory sector organisations (e.g. Age UK South Gloucestershire, Age UK Bristol, Alliance Homes, British Red Cross, NHS, and South Gloucestershire Council) to design and deliver a new approach to non-regulated adult social care and community-based support to speed up discharge from hospital and/or avoid readmission
  • Manage the hospital link worker project that supports speedier discharge from hospital
  • Manage the Welcome Home project that delivers support in people's homes on discharge from hospital and/or works to prevent readmission
  • Manage team leaders
  • Manage project budgets
  • Set up and implementing effective referral pathways
  • Set up and implementing robust recording and monitoring systems
  • Compile reports to submit to funders
  • Monitor progress and provide learning to the steering group so that a future business model can be agreed and developed collectively
  • Set up and run an operational working group to include partner organisations
  • Deliver direct services to clients if required
  • Identify and pilot the use of assistive technology to support delivery
  • The role is based in our Kingwood office with some travel to other locations (including Southmead Hospital) and home working as agreed. This is a Fixed Term Contract until 30th November .

    What we need from you

  • Ability to design, plan and execute a project
  • Ability to set and deliver outcomes
  • Ability to establish and maintain positive and consistent relationships with employees, volunteers, and professionals
  • Excellent communication skills
  • Excellent demonstrable organisational skills
  • Effective time management skills
  • Qualification and 3 years' experience in social work, health or similar
  • Adults Safeguarding training within the last 3 years (Desirable)
  • Supervision training. (Desired)
  • Qualification in counselling/mentoring/coaching. (Desired)
  • Experience working in a management role for 2 years in a related setting and role (for example, social care, care settings, therapeutic environment)
  • Proven experience of using a person-centred approach with service users
  • Proven understanding of mental health and barriers to accessing services
  • Proven understanding of the wider determinants of health
  • Experience of line management and supervision
  • Proven experience of leading, motivating and supporting a team
  • Experience of working with and/or managing volunteers (Desired)
  • Experience of using a management information system
  • Experience of setting up new projects (Desired)
  • Experience of managing projects
  • An understanding of community development. (Desired)
  • Experience of managing budgets. (Desired)
  • Experience of managing services for vulnerable adults at risk of harm. (Desired)
  • Knowledge of South Gloucestershire care and/or VCSE providers
