Description
An exciting opportunity has arisen for a Band 6 Caseload Holder to join North Telford Community Nurses. The post is part-time 30 hours per week, with hours that are flexible.


The position is to caseload manage a group of community based patients offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential, functional independence and good patient outcomes.


The caseload holder will take responsibility for the Immediate and Planned Care to patients in the community neighbourhoods, ensuring service cover from 8am to 6pm 365 days a year and operate effectively providing nursing services.
