    Job summary

    Job Title:Primary Care Nurse (Haringey Integrated Services)

    Hours of work:Full time ( hrs)

    Remuneration:£33,000-£36,000 per annum (based on prior experience)

    Contract length:Permanent

    Responsible to: Practice Manager / Service Manager / Clinical Lead

    Accountable to: Senior Integrated Services Manager

    Line Manager: Integrated Service Manager

    Main Location: Working with General Practice in Haringey with some outbound visits to Care Homes, Supported Accommodation or Home Visits.

    You may be asked to work on site at any practice within the Haringey Borough at the needs of the integrated service.

    Office Location: Haringey GP Federation Offices (48 Station Road, Wood Green, London, N22 7TY).

    We are looking to recruit nurses who want to work in Primary Care whilst further developing their career. This is an exciting and varied career, requiring autonomous practice whilst being part of a multidisciplinary team.

    Within this post you will work closely with the clinical leads to provide physical health checks and clinical reviews for Haringey patients with a variety of long-term conditions and associated comorbidities, providing an invaluable and lifelong impact to their lives.

    Job responsibilities

    Main responsibilities:

    To work in line with Haringey GP Federations organisational policies and procedures.

    To deliver Nurse led clinics according to your clinical competency, general practice, and service needs.

    To independently assess patients, using clinical assessment skills and clinical judgement to complete the physical health check and action any onward reviews or tests to GP practices.

    To review and propose alterations, when clinical necessary, in patients current treatments plans

    To promote a culture of shared decision making to support patients through decision making processes.

    To promote a seamless service of care for patients through liaison with all members of the healthcare team within primary and secondary care.

    To support patients and junior staff with clinical knowledge and experience.

    Work in accordance with internal administrative systems relating to but not limited to the management of clinical data, in line with GDPR.

    To promote and provide evidence-based care that complies with the relevant legal and policy frameworks.

    To work collaboratively with multidisciplinary teams across a range of care settings.

    To always respect patient confidentiality and not divulge patient information unless sanctioned by the requirements of the role.

    Experience

  • Demonstrate practical experience of staff/student supervision.
  • Experience of conducting physical health checks
  • Experience of working with patients that need LTC management - including Diabetes, COPD, Asthma, Hypertension and CKD
  • Experience in mental health nursing and risk assessment and development of management plans
  • Experience in health promotion, monitoring of wellbeing and psychoeducation for patients with long-term conditions and/or mental health conditions
  • Experience conducting health checks with primary or secondary care
  • Experience of working in General Practice
  • Experience of working with EMIS Web
  • Qualifications

    Essential

  • Registered Nurse (NMC)
  • First degree in Adult Nursing or Mental Health Nursing
  • Desirable

  • Qualification General Practice Nursing
  • Evidence of completion of Educator programme (Mentorship or SSSA)
