    Job summary

    An exciting opportunity has arisen for a full-time Band 6 liaison mental health nurse to join the Tower Hamlets Mental Health Liaison team, a multi-disciplinary team with expertise in adult and older adult mental health care.

    The team provides compassionate, person centered and non-judgmental care to individuals presenting to the A&E department with mental health related problems. Assessments, care and treatment are also provided to inpatients at the Royal London Hospital and St Bartholomew's hospital who have mental health needs.

    You will work as part of our Adult Ward team and play an active role in improving the care of people with mental health problems admitted to medical wards at the Royal London Hospital. The role at times will also include working across the emergency department when demand arises.

    Main duties of the job

    You will have excellent clinical skills, be able to demonstrate an ability to complete comprehensive mental state examinations, thoroughly assess risk, formulate safe and effective management plans and document this clearly. You should have experience of mental health nursing, including crisis care in a variety of settings including the community, you will be able work autonomously with awareness of when to seek advice.

    You will hold a sound working knowledge of common mental health disorders, the Mental Health Act and Mental Capacity Act. A sound knowledge of the interface between physical and mental health is essential as well as experience of working with older adults, in any setting.

    Main duties include:

    o To provide rapid response to referrals to ensure timely assessment within currently commissioned targets.

    o Be responsible for the full mental health assessment of patients presenting with a wide variety of clinical conditions within the acute hospital.

    o To devise and deliver interim management and care plans to patients in the ED and on the acute medical wards. To liaise and refer on to other agencies across care providers in both community and inpatient settings.

    o To work with partner agencies to develop plans of care that will prevent admission to hospital. Where admission is unavoidable, to provide or signpost to early intervention to facilitate early discharge.
