Are you an empathetic, patient individual, who wants to make a difference to the lives of vulnerable people? Prospectus is delighted to have partnered with Healthy Living Projects (HLP), a well-established organisation in East London that has been delivering crucial services to the local community for over twenty years.

They are now looking for a Health Advocate on a fixed-term basis for an initial period of 12 months to join their Welcome Project team.

Based in the purpose-built centre in Ilford, the Welcome Project provides advice and guidance on a range of issues associated with homelessness; practical support; support with entering the job market; a nurse led clinic; counselling; alternative therapies and a chiropody clinic.

As a Health Advocate you will work to improve both the mental and physical health of those who access HLP's various services, helping to enable homeless and vulnerable adults to live independent and fulfilled lives.

As part of this role you will work both one-on-one with clients, providing advice and guidance, developing support/actions plans to help them tackle difficulties they may be experiencing, but also organising group sessions and practical workshops that address barriers to living independently including life skills, employability and relationships.

Working alongside the Welcome Project team, you will be involved in the development of new policies, procedures and initiatives, and contribute towards the development and implementation of strategies to tackle homelessness.

Partnership working is key in this role; you will regularly review support plans and liaise with other professionals such as social workers, probation officers and psychiatric nurses where necessary.

You will also develop and maintain links with key service providers, and make referrals to other agencies, both statutory and voluntary, where issues raised require more specialised help.
