    Description

    We are looking for an Enablement Worker to join a new 'Home First' team who will work closely with our Rehab assistant practitioners. You will work with individuals in their own home, to maximise patient independence, preventing admission to hospital and supporting the rehabilitation programmes set up by the rehab assistant practitioners.

    This is an exciting opportunity to work within an established Integrated team in a community based setting focusing on facilitated discharge and prevention of admission.

    You will be expected to travel efficiently and effectively between various work locations and patient residences, so a full current driving license and use of a vehicle is essential.

    In order to be able to carry out the duties of this post effectively and safely, candidates must have at least one year's experience in a care setting. Applicant must have good communication skills – both verbally and in writing. SVQ Level 3 in Care, or a willingness and ability to work towards this qualification within an agreed time scale.
