    Job summary

    The Advanced Clinical Practitioner(ACP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients.

    The post holder works at the level of advanced clinical practice and has developed and can evidence the underpinning competencies applicable to the specialty or subject area, , the knowledge, skills and behaviours appropriate to the healthcare professionals (HCP) setting and job role.

    The role provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.

    Main duties of the job

    An ACP is autonomous in making decisions based onassessment, diagnosis and interpretation of test results and will initiate,evaluate and modify a range of interventions whichmay include prescribing medicines, therapies, lifestyle advice and care.

    Filey Surgery is currently recruiting for a friendly, committed &motivated Advanced Clinical Practitioner (ACP) to work within our Medical Team

    Filey Surgery provides high quality healthcare for around 9,000 patientsin Filey & its surrounding areas. We are a multi-disciplinary team of GP's,Advanced Clinical Practitioners, Nurse Practitioners, Practice Nurses, HCA's,Pharmacists, Dispensers, Medical Secretaries, Administrators &Receptionists.

    The role of ACP will includeresponsibility for triaging, assessing, examining and diagnosis of patients,predominantly for our acute patient presentations.

    Themedical team works closely to manage the needs of our patients, supporting eachother to reach the best outcome for our patients.

    Learningand development is at the heart of our practice. We are a training and teaching practice forGP Registrars and HYMS Medical students as well as supporting student nurses intheir development. We are also keen totrain and develop our staff ensuring they maximise their potential.

    Job responsibilities

    Thefollowing are the core responsibilities of the Advanced Clinical Practitioner.

    a.Practise in compliance with their respective code of professional conduct andwithin their scope of practice, being responsible and accountable for theirdecisions, actions and omissions at this level of practice.

    b.Demonstrate a critical understanding of their broadened level of responsibilityand autonomy and the limits of own competence and professional scope ofpractice, including when working with complexity, risk, uncertainty andincomplete information.

    c.Act on professional judgement about when to seek help, demonstrating criticalreflection on own practice, self-awareness, emotional intelligence, andopenness to change.

    d.Work in partnership with individuals, families and carers, using a range ofassessment methods as appropriate.

    e.Demonstrate effective communication skills, supporting people in makingdecisions, planning care or seeking to make a positive change.

    f.Use expertise and decision-making skills to inform clinical reasoning.

    g.Initiate, evaluate and modify a range of interventions which may includeprescribing medicines, therapies, lifestyle advice and care.

    h.Exercise professional judgement to manage risk.

    i.Manage patients presenting with a range of acute and chronic medicalconditions, providing subject matter expert advice.

    j.Implement and evaluate individual specialised treatment plans for chronicdisease patients.

    k.Identify, manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health.

    l.Be a professional role model for the organisation.

    m.Actively engage in peer review to inform own and others practice, formulatingand implementing strategies to act on learning and make improvements.

    n.Actively seek feedback and involvement from individuals, families, carers,communities and colleagues in the co-production of service improvements

    o.Critically assess and address own learning needs, negotiating a personaldevelopment plan that reflects the breadth of ongoing professional developmentacross the four pillars of advanced clinical practice (clinical practice,leadership and management, education and research).

    p.Provide chronic disease clinics, delivering patient care as necessary, referringpatients to secondary/specialist care as required.

    q.Maintain accurate clinical records in conjunction with extant legislation andto ensure SNOMED CT codes are used effectively.

    r.Critically appraise and synthesise the outcome of relevant research, evaluationand audit, using the results to underpin own practice and to inform that ofothers.

    s.Develop and implement robust governance systems and contribute to practicetargets (QOF, etc.), complying with local and regional guidance.

    t.Chaperone patients where necessary

    u.Support the team in dealing with clinical emergencies.

    v.Support the team with all safeguarding matters in accordance with local andnational policies.

    w.Understand practice and local policies for substance abuse and addictivebehaviour, referring patients appropriately.

    x.Deliver opportunistic health promotion where appropriate.

    Inaddition to the primary responsibilities, the Advanced Clinical Practitionermay be requested to:

    a.Support the IPC lead, ensuring compliance, and undertaking audit as necessary.

    b.Monitor and ensure the safe storage, rotation and disposal of medicines.

    c.Assist in the support of the wider clinical team, providing guidance whennecessary, acting as a mentor to students and newly qualified staff.

    d.Participate in local initiatives to enhance service delivery and patient care.

    e.Support and participate in shared learning within the organisation.

    f.Develop an area of specialist interest, taking the lead within theorganisation.

    g.Continually review clinical practices, responding to national policies andinitiatives where appropriate

    h.Participate in the review of significant and near-miss events applying astructured approach , root cause analysis (RCA)

    i.Commensurate with their role, support the development of clinical serviceswithin the organisation, liaising with external agencies and professionalorganisations as required.

    j.Develop practice administrative and clinical protocols in line with the needsof the patient and current legislation.

  • Qualified healthcare professional
  • Educated to Master's degree as detailed within the HEE document titled Multi Professional Framework for Advanced Clinical Practice In England
  • Qualified independent prescriber - V300
  • Registered on their appropriate professional register
  • Desirable

  • Teaching qualification

  • Clinical Knowledge and Skills
