- Carry out assessments risk/initial/comprehensive to inform treatment journeys. Carry out interventions relevant to individual client needs including 1:1 sessions and group work/programme facilitation.
- Research local resources to support referrals and work in partnership with other agencies to ensure that all clients' diverse needs are met.
- Coordinate and work alongside Health care to monitor and manage client health and minimising risks of harm
- Develop pathways supporting and co-ordinating client engagement to address their diverse support needs.
- Motivate client to engage with support services identified in care plan and risk assessment.
- Raise awareness and standards of the needs of diverse service users, promoting a person centred approach.
