    About the role

    :

    Our community Reablement service delivers a successful short term therapy assessment and care program, that supports our residents to live well in their own homes, reducing their long terms care needs and maximising their health and wellbeing. Due to the pandemic the service has actively responded and adapted to support people through the implementation of a rapid Discharge to Assess HomeFirst model.

    As an experienced manager or a senior care worker in a CQC regulated service, you will work closely with health and social care partners across an integrated care system and be at the foreground of exciting change and transformation. We have an innovative and vibrant approach to enhance and broaden the services that support people leave hospital, and with our partners design and support the development & delivery of a social care crisis response model, enabling people to remain in their homes, avoiding the need for admission to hospital or residential care settings.

    We are a fast-moving responsive service that moves as one team and are seeking a dedicated, passionate, and self-starting individual, who will be required to hold or working towards, a Health & Social Care qualification at NVQ level 4-5. So that the standard of service remains compliant and well led in delivering effective care and support, within the CQC regulatory and Safeguarding frameworks. You will evidence your experience and confidence in managing people and teams, placing leadership at the core of everything you do.
