    We are looking for a part time Occupational Therapist to join our band 6 rotation made up of Primary Care Home, Neurology and Frailty. This post is for hours.

    The Primary Care Home Team - is a multidisciplinary team of Nurses, Occupational Therapists, Physiotherapist, Assistant Practitioners, Paramedic, Phlebotomist, Healthcare Assistants working closely with GP's, Social Services and other community providers to provide an excellent standard of person centered care to adults in their own homes, residential care or 'beds with care', supporting people to self manage whenever possible. As a Occupational Therapist within the team you will lead on on the reablement of patients to ensure the patients needs and goals are met, regularly liaising with team members to progress rehab programs relevant to changes in the patients condition, this will range from reacting to patient in crisis, palliative patients, to supporting discharge and admission avoidance. You will have a focus on optimising independence in activities of daily living.

    Main duties of the job

    The Integrated Community and Neurology Team - The Integrated Community Neurology Team, supports patients with a range of neurological conditions and consists of Specialist Nurses, Physiotherapists, Occupational Therapists and Rehabilitation Assistant Practitioners. This unique team works with people with acquired neurological conditions such as Stroke and Spinal Cord Injuries to provide goal centered rehabilitation in both clinic and home settings, as well as long term conditions such as Multiple Sclerosis, Parkinsons and Motor Neurone Disease to provide ongoing reviews supporting them throughout their journey from initial diagnosis, including long term condition management and palliative care. For all conditions our aim is to improve patient quality of life and maximise their independence.

    The Frailty Prevention Team is a multidisciplinary team consisting of Occupational Therapists, Physiotherapists, Assistant Practitioners, Exercise Professionals and Nurses. The team provides admission avoidance, Frailty Prevention Assessments,

    Falls Assessment, Home Hazard and Environment Assessment, Equipment Provision, Strength and Balance training. The Team will endeavor to advise patients, relatives and carers on how best to assist patients to remain well and manage their everyday living. Our aim is to maximise their independence, achieve their goals and to support them to remain safe and well at home.

    To apply specialist skills and knowledge to manage acaseload of complex cases using evidence based / patient centred principles toassess, plan implement and evaluate interventions in order to establishprofessional competence to practice as a senior Occupational Therapist.

    Using a standardised approach but with a high degreeof professional autonomy and accountability, work across Health, Social Care,Voluntary and other health providers and agencies, to identify a defined groupof patients with complex needs to provide a single plan of care co-producedwith the patient

    Using specialist clinical practice skills assess thephysical and psychosocial needs of a defined client group.

    Critically analyse clinical data and information toinform diagnosis and, where appropriate, order investigations and/or instigatetherapeutic treatments to inform clinical decision making and improve healthoutcomes.

    Provide individualised care plans for a complexpatient caseload and ensuring that any delegated workload is completed by staffwith the appropriate competencies.

    Analyse complex patient situations and needs, eitheras part of a complex caseload or to provide advice and support to others withinthe Primary care network.

    Use assessment tools/skills that will ensure anappropriate level of therapeutic intervention so that patients who present withhighly complex needs are timely referred to the appropriate specialist servicesor interventions.

    Be accountable for practice taking every reasonableopportunity tosustain and improve knowledge and professionalcompetence and, ensure that all aspects of professional behaviour as requiredwithin the Code of Ethics and Professional Conduct for Occupational Therapists(Royal College of Occupational Therapist) and Standards of Proficiency forOccupational Therapists, (Health and Care and Professions Council, HCPC) arefollowed at all times.

    Maintain legible, accurate and contemporaneous patientrecords in accordance with ECCH Policy and the Royal College of OccupationalTherapy Standards for Record Keeping.

    Assist with the investigations of complaints,participate in the risk management process, critical incident reporting,evaluation, dissemination and change in practice.

    Be responsible for understanding, following andimplementing ECCH policies and procedures, and influencing working practices tosupport this accordingly.

    Contribute to the clinical governance agenda throughparticipation in clinical risk assessment and management, clinical audit.

    To participate within the wider integration acrossTherapies and wider Community Care teams including Primary, Social Care, andall other provider organisations to provide high standards of care to patientsand where appropriate avoiding unnecessary admission to secondary care or caresettings.

    Assist with the implementation of pathways andguidelines to develop and deliver new innovative models for service delivery,ensuring a leading edge approach to service development in-line with evidencebased practice, national guidance at a local level.

    Contribute and develop systems to monitor, evaluateand audit service quality mechanisms to meet nationally and local targets.

    Work with ECCH Colleagues, and other partner agenciesand stakeholders including the acute trust to contribute to the development anddelivery of new innovative models of service delivery, ensuring a leading-edgeapproach to service development in-line with evidence based practice.
