    To promote choice in the delivery of rehabilitation, care, and support. Reviewing and making suggestions to enhance the individuals experience. Enabling concerns and complaints to be raised freely by the individual and/or their representatives

    Rehabilitation Assistant

    Ipswich

    Salary: £ per hour plus 30% for weekend and unsociable hours (8pm 6am)

    MAIN PURPOSE:

    The purpose of this position is to carry out high quality holistic person-centred care and support to service users, to reach their full rehabilitation potential.

    MAIN TASKS AND RESPONSIBILITIES

    To participate fully in the Care Plan process in the role of Associate Key Worker including assessment, implementation, and evaluation

    To promote choice in the delivery of rehabilitation, care, and support. Reviewing and making suggestions to enhance the individuals experience. Enabling concerns and complaints to be raised freely by the individual and/or their representatives

    To attend to and assist clients in achieving a high standard of physical care, including diet, hygiene, and appearance. To provide a high standard of personal care to clients in a sensitive and respectful manner, and help them maintain their morale

    To monitor the physical and emotional well-being of all clients in order to be aware of changes and to alert senior colleagues as necessary

    To work with therapists/team members to ensure optimum cognitive, social, emotional, physical, and functional recovery

    To ensure that rehabilitation goals, strategies, and procedures are implemented and reinforced throughout the 24-hour period

    To observe strict confidentiality in respect of any information regarding clients or information relating to the running of the Care and Rehabilitation Services

    To promote an organisational culture where staff are empowered to challenge inappropriate behaviour and unsafe practice using appropriate organisational processes including the internal whistle-blowing policy

    To seek feedback on all incidents reported by the post holder through DATIX

    To participate openly and honestly in any investigation regarding care delivery

    Where there has been harm to a person in our care to work with the line manager to ensure that the service user and /or their representative are informed of the facts surrounding any incident (see explanation re Duty of Candour in Incident and Near Miss Reporting Procedure)

    To ensure accurate reporting and documentation

    To be an active and co-operative team member. Challenging practice as appropriate and supporting new staff

    In order to facilitate the safe and smooth running of the Service, employees are expected to maintain a safe working environment, maintain service user confidentiality, and occasionally take on reasonable additional tasks

    Person Specification

    Qualifications

    Essential

  • S/NVQ /QCF Level 2 or other relevant subject/training
  • Willing to study S/NVQ Level 3 (or equivalent) in Health & Social Care and committed to taking part in QCF studies with the intention of achieving Level 3
