Due to the Admission Prevention and Discharge Support service expanding, we are currently recruiting recovery coordinators to work across Norwich, North Norfolk, and Yarmouth & Waveney.
Tell me a bit more about theAdmission Prevention and Discharge Support Service

  • Admission Prevention supports those at risk of being admitted to hospital, or being discharged from hospital, due to mental health needs.
  • Community Outreach operating across Norfolk, 7 days a week, including bank holidays, between the hours of 09:30 and 20:00.
  • Staff shifts are set on a 7week rolling rota, flexibility is welcomed and working hours/shift patterns are discussed at the interview.
  • Staff work across two localities; South & West Norfolk and Central, North and Great Yarmouth & Waveney staff are allocated to a locality, however, are expected to travel wherever the demand for support is therefore, travel across Norfolk and Waveney is expected.

Tell me more about the job.

  • A typical day within this role consists of visiting vulnerable adults with mental health needs in the community or in an inreach setting to provide a variety of support including, emotional support, social inclusion activities, signposting and supporting to engage in services in the community.
  • You will be responsible for holding and managing a small caseload.

As Recovery Co-Ordinator, you will be required to;_

  • To offer a creative, flexible approach, with the aim of providing a service that will reduce a client's level of vulnerability, increase their control over their life, and work with them towards a recovery that is meaningful to them.
  • To provide support to people with mental health problems, enabling them to achieve their optimum level of independence.
  • To work in partnership with the people we support, building on their strengths, enabling them to engage with services that would be beneficial to them in sustaining independence and achieving personal goals.
  • To work with colleagues to provide a service to people with complex needs, including mental health, substance use problems and selfharm.
  • To work closely with agencies and stakeholders, practicing excellent communication to manage risk and follow safeguarding procedures.

The ability to drive and access a car for business use is essential.
