They will facilitate implementation of rehabilitation plans and review patient progress to enable a safe discharge.

They will utilise their skills to complement the existing clinical disciplines in the Team, to enhance the team's skills and knowledge development.

They will also lead on the future development of the Service providing professional advice and expertise drawn from their previous experiences to enhance the overall quality of care across the Service.

You will also be required to ensure delivery of high-quality patient centred care measured by clinical effectiveness, patient safety and patient experience as well as ensuring clinical records are accurate and maintained in a timely way.

The post holder will be flexible to support patient appointments by telephone, virtual and face to face clinics whilst maintaining the same standard of high-quality care in each setting.

The Service strives to learn and improve delivering high quality care across a highly diverse patient group and a relatively new long-term condition.

It has developed new means of supporting patients through sharing of best practice with other Post Covid Services and has introduced new pathways to support this highly complex patient group.

In addition the Service works in partnership with University Hospital Birmingham Foundation NHS Trust and is supported by Birmingham & Solihull's Chronic Fatigue Service.
