- Assessment of service users and carers with complex needs in relation to home environment.
- Experience in Moving and Handling is essentialas well as Single handed care knowledge.
- Assess and treat patients requiring ward-based rehabilitation and assessment for discharge planning.
- Working along side the Team Manager. Visits to residents homes will be required. To ensure collaboration with our partner agencies.
- Provide expert advice and intervention regarding complex manual handling situations.
- Undertake assessments and documentation associated with Blue Badge applications.
- Experience of functional assessment experience of working with people with disabilities/vulnerable people.
- Providing expert advice, and assessments for equipment and home modifications.
- Recommending adaptations.
- An awareness of the importance of safeguarding vulnerable people, plus an understanding of the importance of dignity & respect when dealing with people.
