An opportunity has arisen to employ a Transforming Care and Accommodation Housing Lead within Cheshire and Mersey Integrated Care System (ICS).

This includes working across 9 Local Authorities and 9 Places (previously CCG's).


As an ICS we require a full time Transforming Care and Accommodation Leadto work with local authority housing departments, commissioners in health and social care, estates teams, Housing Associations, and expert support providers.

Lead on the implementation of theTransforming Care Housing Plan which is in progress.


Generate pipeline of supported housingdevelopments, linking with local authority housing and Housing Providers, sharing aggregated needs, and identifying opportunities in key areas where specialist supported housing is needed within Cheshire & Merseyside.


Improving links to providers of housing and developers, and funding bodies such as NHSE and Homes England to maximising capital opportunities and link in with local development planning.


Support projects that provide suitable housing for people with Learning Disabilities and Autism working with commissioners and service providers to shape a market of capable providers.

  • The existing Transforming Care is made up of health and Social Care professionals.


The Transforming Care Programme requires the expertise of a housing lead due to the wide range of housing needs required for our population group.


Many elements of Transforming Care are interlinked with Social Care and housing in the delivery of the Transforming Care agenda which has been signed up to by the Associationof Directors of Adult Social Services(ADASS).

Job Purpose:


  • To lead on the development of key housing development projects that meet the commissioning needs of the area in line with meeting Transforming Care requirements across Cheshire and Merseyside, to deliver sustainable service improvements based on the terms of the planning for development and service specification and contract.
  • Facilitating and building relationships and effective communication with all provider and commissioning organisations in and around Cheshire and Merseyside, harmonising cultures, managing conflicts of interest, and achieving common understanding, to ensure there is a constructive and open dialogue about the development of effective working to deliver the agenda.
  • To lead on the development of the Learning Disabilities and Autism Programme Housing and Care requirements and planning arrangements to deliver a strategic approach and housing development projects that will support capacity and demand planning, management and redesign where it promotes and strengthens the capability to deliver quality care into the community as outlined in The Building on Strengths Document.
  • Work across housing, health and social care on specialist needs housing and care development. This includes sharing information on individual and cohort needs in an accessible and confidential way to develop provider interest in supporting commissioning intentions in the area. To be responsible for delegated activities and work streams within the service redesign project including writing specific cases for change/service specifications/action plans and/or proposals and leading these to delivery where appropriate. Lead and facilitate on specific areas of work as appropriate in support of the overall management of service development with clinical leads and professionals which will include the submission of bids for funding to support project workstreams. Actively develop and manage project plans to meet specific areas of complex needs. Deliver progress reports on effectiveness of meeting Learning Disabilities and Autism Housing needs, delivery of proposals and related services, working closely with Place colleagues, Transforming Care Programme Lead and partners. Regular audit of implementation, highlighting variance from plan and providing support to group leads as necessary.
Specific responsibilities


To support the development and improvement of commissioned housing with care services in line with national and local policy, ensuring work-streams are delivered within defined parameters.

The post holder will lead and facilitate change by working with multiple stakeholders across the Health and Social Care community in Cheshire and Merseyside to:
