Your new role
We are looking for an enthusiastic and dynamic Clinical Nurse Specialist (CNS) or Clinical Paramedic Specialist (CPS) to work as part of our Hospice Outreach Team in providing specialist palliative care support to our patients who are living in the community, both in their own homes and within care homes/nursing homes.
Our team are the initial point of contact for all new community referrals and all telephone calls coming into the service. You will be part of a rota of clinicians covering a 7 day a week service, offering advice and care to our patients both in clinic and home settings, as well as remotely via telephone and videoconferencing support.
Our team care for patients who have a life limiting illness and our focus is on individualised care, focusing on improving quality of life and achieving patient centred goals. We care for people with long term conditions including heart failure, respiratory disease, frailty and neurological conditions, as well as those with an oncological diagnosis and our team strives to work alongside other community teams and specialists to optimise patient experience and improve our team knowledge and development.
To succeed in this role
You will have proven community or in-patient palliative care experience at Band 6 or 7, or have equivalent level of experience of working in the following related areas: Paramedic, General Medicine, Neurology, Emergency Medicine, Frailty, Respiratory Care, Cardiology or Oncology.
We are looking for a motivated clinician with the ability to work as an independent practitioner whilst being part of a responsive and resilient team. You will bring with you a wide range of experience in mentoring and supporting individuals and teams and possess advanced communication skills and excellent presentation skills (both oral and written).
