    Job summary

    We have an exciting opportunity for the right person to join our team and play a major role in the leadership team leading the Pershore & Upton, Malvern and Evening & Nights Community Nursing Neighbourhood Teams.

    The role entails the operational management of these 3 Teams which includes the line management of six Band 7 direct reports and Band 4 PA; budget management; Quality and performance monitoring and service improvements.

    The post holder will need to take part in system calls regarding patient flow and support with the escalation of delays where needed.

    Leading and monitoring Innovations around End of Life, Virtual Wards, OT pilot, ACPs, intensive support and catheters are also key work-streams.

    This role includes being part of the on-call rota for the Trust.

    Main duties of the job

    You will work closely with the DeputyAssociate Director and fellow Integrated Community Service Leads as part of ourSenior Management Team.

    Accountableto the Deputy Associate Director you will support strategic leadership toclinical services operationally on a day-to-day basis and will be aligned withthe following services: Neighbourhood Teams, MSK / APP Services, and ourHousing, Adaptations and Equipment Services.

    Thepost holder will be involved in an extremely exciting and challenging period ofservice development with the further roll out of the Ageing Well Agenda,enhanced models of frailty care and developing closer working and operationalalignment with partner organisations. You will require energy and a commitmentto quality improvement across this diverse portfolio.

    Job responsibilities

    The successful applicant will work within thebeautiful location of Malvern, Pershore and Upton, and join an experiencedsupportive team of Service Leads. The role is full time, with on-callmanagement duties to cover weekend and the out of office period during theweek. A professional health qualification and up to date registration is arequirement.

    As a Service Lead you will support therealisation to build a fully integrated community offer for our patients inline with the Fuller Report. We work closely with General Practice and othersystem partners to:

  • blurthelines across local health and care organisations - integrating management andleadership to enable a more 'joined up' operational solution - ultimatelybenefitting our patients.
  • optimise the potentialto jointly implement the Fuller recommendations across our PCN footprints -focussing on optimising joint resources across the PCN footprint (rather thanworking within organisational silos).
  • secure effective ways tounlock potential efficiencies in relation to holistic care provision acrossorganisations - with the patient at the heart of everything we do.
  • improve patientexperience and ultimately result in a more effective, efficient offer (byjointlydelivering outcomes,reducing visits across organisations, andsupporting the green strategy).
  • optimise local assets(via District Collaborative engagement) to promote local self-help solutions.

    Qualifications

    Essential

  • A current professional qualification (RMN, RGN, OT, Physiotherapist)
  • Registered to practice in the UK.
  • First line management qualification or equivalent experience
  • First degree or equivalent in health care subject
  • Evidence of on-going CPD
  • I understand that I will be asked to provide original certificates to verify my qualifications at point of offer

  • Masters Level Qualification or working towards
  • Experience

  • Significant post graduate leadership experience in core areas
