Description

Southwell ward is a busy 33 bed ward where no day is ever the same.

Our team of discharge coordinators track our patients throughout their ward stay to ensure timely and safe discharges or transfer on to speciality wards.

Our discharge coordinators support the Nurses by completing non clinical tasks such as booking Transport, liaising with diagnostic specialist or talking to relatives.

The discharge coordinator works closely with our integrated discharge team and social care colleagues.
