    Job summary

    Newhall Surgery is looking for a motivated, empathetic, organised 'people person' with a positive attitude, good listening and communications skills to join our Clinical Administration Team.

    Previous experience of working within a medical environment would be advantageous. This is a varied role with much of the work undertaken being of a highly confidential and sensitive nature.

    This role involves liaison with patients, carers and external agencies to provide support to our vulnerable patients and those who are needing additional advice and guidance.

    This position requires excellent people skills, an understanding of local pathways and support networks; it is a demanding but very rewarding role.

    Main duties of the job

    The successful candidate will be a member of the Clinical Administration team working closely with GPs and external agencies. The post holder will act as a point of contact for patients and carers, answering queries and signposting to relevant local services. They will support the practice with caseload management and identification and will be involved in the coordination of MDT Meetings.

    The role will be responsible for various areas of data management including data analysis, auditing, coding and ensuring patient records are correct and up to date.

    The role of the Patient Care Coordinator goes beyond administration, requiring engagement with patients and to work within a network of local providers and organisations to ensure patients have access to the care they need.

    The post holder would also be the CST, Adult and Children Safeguarding and Palliative Care administration lead and coordinate patient care working alongside multi-disciplinary agencies.
