    Job summary

    An exciting opportunity has arisen for a practice nurse developmentpost within our large NHS GP practice which provides General Medical, Studenthealth, Homelessness service along with other specialist services across ourseven sites in and around Liverpool City Centre.

    Initially a band 5/6 dependent on previous experience, trainingwill be provided over a 12-year period, during which time development will includea 2-year GPN fellowship/Preceptorship programme.

    This post may be particularly attractive opportunity tonewly registered nurses or those nurses just moving to a general practice setting,The training and preceptorship programme will include cervical cytology,immunisations and long-term condition management.

    This role requires consolidation of registrant standards ofcompetence and developing confidence to work alone without direct supervision,undertaking and reporting on autonomous decisions made in practice.

    This role requires a developing knowledge of practicenursing in the broadest sense, and excellent interpersonal and communicationskills to support patients with a wide range of conditions to understand and,where possible, take on self-management of their condition.

    Main duties of the job

    Activelyengage in a programme of induction and preceptorship which will involve online,classroom and practice-based learning, with regular competency assessments.

    Abilityto assess patients, considering their physical, mental and social statesalongside the impact of their environment and social support available to themand negotiating care plans that are person centred and focused on self-carewith clear objectives.

    Using arange of assessment tools pertinent to the patients needs to inform theassessment and assess risk for both patients and staff.

    Articulationof risk and strategy for risk management.

    Knowledgeof a broad range of conditions, local care pathways and evidence-basedmanagement experienced by patients in general practice settings. This willinclude long term conditions, for example diabetes, coronary heart disease,heart failure, hypertension and stroke and COPD.

    Anunderstanding of the presentations of multiple pathology, depression andanxiety states and frailty predominantly in older people.
