Are you a dedicated and compassionate Social Care Practitioner looking for an exciting
opportunity to make a difference in the community? West Berkshire Council is currently seeking a
motivated individual to join our Adult Social Care Service as a Social Care Practitioner.

As a Social Care Practitioner, you will work closely with the Team Leader to provide personcentered
support to vulnerable adults in our community. Your main responsibilities will include
assessing care needs, developing and implementing care plans, coordinating support services,
and advocating for service users' rights and wellbeing. This is a varied role that requires strong
communication and interpersonal skills, as well as the ability to work effectively within a multidisciplinary
team.

  • A relevant qualification in social care or a related field
  • Experience working within adult social care, preferably in a similar practitioner role
  • Excellent assessment and care planning skills
  • Knowledge of relevant legislation and best practice guidelines
  • Strong communication and advocacy skills
  • A compassionate and empathetic approach to supporting vulnerable adults
