Job overview:
The Torpoint and Saltash Community Nursing Team due to the increase focus on Place Based Care are now looking to appoint a Qualified Nurse Associates.
This is an exciting and fulfilling role and you will need to be able to demonstrate a real enthusiasm for nursing people in the community and their own home.
The team currently works from two bases and provides skilled evidenced based care in the Saltash, Torpoint and the surrounding area.
This offers the applicant variety on a weekly basis in both areas and includes beautiful countryside and coastal views.
You should also have an understanding of the changing climate of care delivery with the ability to travel independently across the area to fulfil the requirements being essential.
Main duties of the job:
The role of the Nursing Associate is to work with the Registered Community Nurses and other members of the multi-professional team, providing and monitoring care and contributing to the ongoing assessment,
as agreed with supervising clinician(s) and in line with the service pathways.
They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi professional team.
This role will support the Community Nursing team with organising the patients on the end-of-life or Continuing Care at Home caseload.
