    ARE YOU ANEXPERIENCED NURSE READY TO STEP UP TO A SENIOR ROLE?

    We are seeking a number of enthusiasticand motivated clinicians to join our ANP team based within seven CommunityHospitals across Leicester, Leicestershire and Rutland. We have developed aclinical pathway delivering medical care for patients and our diverse caseloadrequires practitioners with comprehensive assessment and medical managementskills.

    This is both achallenging and rewarding role which will give you scope for higher leveldecision making. Ourpatient cohort is increasingly complex due to the need to facilitate transferfrom acute trusts for individuals who are medically fit for discharge and toprevent admission from home to the secondary care sector. You will beresponsible for assessment, diagnosis and development of treatment plans. Thewards receive onsite patient review twice a week from a senior doctor and outsideof this time the ANP will be the senior clinician on site, leading the wardround and taking clinical decisions alongside the MDT.

    Therole will bring high levels of responsibility and will also provide theopportunity for autonomous working in this challenging, interesting & changing environment.

    Cross coverworking may be expected & a base will be discussed at interview. The contracted hours will be worked flexibly between - Monday to Friday.

    It is essentialthat you hold a UK driving licence & have access to a vehicle. Free parking isavailable on all sites.

    Main duties of the job

    MAINDUTIES OF THE JOB

    Towork closely with consultant geriatricians, consultant nurses, a team of ANPs,nursing and allied health professionals in both primary and secondary care, toensure patients receive investigations,interventions and treatment with minimal avoidable delays, whilst improving thequality of care.

    Towork autonomously assessing individual patient needs, initiating investigations,determining differential diagnosis and initiating appropriate holistic,evidence-based treatment with the development of a personalised clinicalmanagement plan, including independent prescribing and appropriate escalationplans.

    Tohave confidence and competence in recognising a deteriorating patient and to initiatea treatment accordingly and appropriately.

    Practiceautonomously in providing assessment, diagnoses and treatment plans for complexpatients including requesting investigations and interpreting results.

    Toput the patient at the centre of all decisions, reinforcing the concept of amore streamlined patient journey, improving the quality of care.

    Toprovide support and teaching to trainee ANPs and medical students.

    Experience

    Essential

  • Extensive post registration experience at Band 7/8a practitioner level.
  • Evidence of research/audit experience.
  • Extensive experience and management of Frail Older Person/Chronic Disease Management/Long Term Conditions.
  • Demonstrate the ability to work in a highly demanding environment as part of an integrated team.
  • Able to work and make decisions autonomously and take initiative.
  • Manage and facilitate staff development.
  • Excellent communication, facilitation and leadership skills.
  • Ability to manage a complex caseload and delegate to appropriate skill mix members of team or refer to other professionals if required.

  • MSc in Advanced Clinical practice or equivalent Masters qualification or verified portfolio of evidence of working towards.
  • Independent Non-Medical Prescriber Qualification (V300).
  • Advanced Communication Course.
  • Evidence of on-going professional development.
