We are seeking to appoint a motivated and dynamic Health Visitor with a passion for improving outcomes for children and their families.

Working in a diverse area you will find that there are lots of opportunities for developing public health work within communities and partner agencies, as well as delivering the core contacts of the Healthy Child Programme.


To deliver the public health role as a SCPHN Health Visitor working with communities and partner agencies and delivering the core contacts within the Healthy Child Programme.


  • To provide health visiting services to families in the community team caseload by delivering the Healthy Child Programme.
  • To manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload.
  • To have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care.
  • To prescribe according to assessed individual need from the Nurse Prescribing Formulary.
  • To manage a team of health visiting skill mix staff.
  • To support a positive learning environment for students


Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.


We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people's own homes and via virtual consultations.


During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.
