    Job summary

    We are looking for an enthusiastic, innovative, experienced Physiotherapist to join our friendly, forward-thinking team providing rehabilitation for a diverse range of people with stroke and neurological conditions within Stockport.

    Main duties of the job

    Based in the community, you will work within a comprehensive multidisciplinary team of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Clinical Psychologists, Nurses, Assistant Practitioner, Therapy Assistants and Admin. You will be welcomed as a valued member of the coordinated and committed team, sharing knowledge, skills and opportunities to learn.

    You will be provide specialist knowledge to facilitate the early supported discharge of stroke patients from the stroke unit, and work with patients with acute and long term neurological conditions to provide rehabilitation in the home environment, and other community settings, improving their quality of life.

    You will support the team through clinical and managerial leadership to promote an efficient, effective service for all patients and carers.

    Support and education is provided with the opportunity to work directly with experienced and specialist clinicians across the team and the chance to contribute meaningfully to a fast-paced and growing team.

    Job responsibilities

    You will undertake comprehensive assessments and use your highly advanced clinical reasoning skills to develop and deliver rehabilitation programmes that support patients to achieve their goals.

    You will participate within and facilitate our weekly MDT meeting, supporting therapists from all disciplines ensuring fully co-ordinated communication and goal-setting. You will provide specialist support and training to all members of the multidisciplinary team

    Your role will be a division of clinical and managerial activities, providing expert knowledge in your professional field, and supporting the team within operational activities and service development.

    As a member of the CNRS team you will have excellent assessment, communication and problem-solving skills and demonstrate an adaptable, flexible approach whilst meeting guidelines and standards.

    Joining the Community Neuro Rehab Service (CNRS) in Stockport, you will see were a supportive, caring and proactive team. Everything we do is aimed at providing exceptional, holistic, person-centred care to people with stroke and neurological conditions

    If successful this vacancy will require an enhanced DBS check at a cost of £ This cost will be deducted from your first Trust salary

    In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.

    Person Specification

    Education & Qualifications

    Essential

  • Degree in PT
  • State Registration
  • Evidence of CPD maintained in a portfolio
  • Clinical Educators/Field Worker course or equivalent
  • Desirable

  • 'M' level or equivalent courses in relevant speciality
  • Knowledge

    Essential

  • Clinical reasoning skills
  • Knowledge of a range of relevant conditions and their management
  • Knowledge of recent NHS and trust developments and their impact on the service
  • Expert Knowledge of the Stroke and Neurology conditions
  • Knowledge of RCP and SSNAP guidelines
  • Knowledge of relevant policies & legislation
  • Desirable

  • Knowledge of Clinical Audit
  • Experience

    Essential

  • Clinical experience as a Band 6 with specialist knowledge of Stroke and Neurological conditions
  • Experience of working as part of a Multidisciplinary team
  • Experience of organising and implementing training programmes for staff
  • Experience of managing tone/ spasticity.
  • Experience in assessment and treatment of complex cognitive problems including mental capacity assessments.
  • Experience of managing change in a service
  • Experience of effective team working across settings

  • Previous Band 7 experience in relevant post
  • Experience of working in inpatient and community settings
  • Experience of student education within the workplace up to graduate level.
