    Are you a Mental Health Nurse who has a passion to work within OPMH and want to join a friendly team to do this?
    The New Forest West Older Persons Community Mental Health Team is based at South Efford House.

    As a community mental health practitioner your journey will take you around the beautiful New Forest and along the coast delivering high quality care and interventions to our patient group.

    Currently the OPMH Transformation team is focusing on the following services:
    *In Reach/Liaison Services to care homes and hospitals
    *Crisis out of hours provision for functional mental health and dementia
    *Functional mental health pathway and care coordinator role

    If you have a keen interest in any of these areas, then now is the time to join and influence the development and delivery of these pathways to help shape the future excellent standard of care for our patients and their carers.

    The supportive leadership team promote the development of our services to provide the best outcomes for our patients and we know that to achieve this we need to invest in our staff through creating a good working environment and providing training opportunities.

    We are also open to flexible ways of working with hybrid working (a mix of face to face and online/telephone assessments) available to help balance your work and home-life.

    Ability to drive and use of a car for work purpose is essential in this role.
