Description
Consultant for Virtual Ward and Acute Frailty Team.

10 PA with on call.

You will be joining a dynamic and enthusiastic team of 12 Consultant geriatricians with complementary skills, and with special interests in acute care, community rehabilitation, movement disorders, stroke medicine, orthogeriatrics, continence, dementia, falls and polypharmacy.

We are also supported by three specialist advanced clinical practitioners and 4 registrars, as well as a diverse and highly valued multidisciplinary team, who share our passion for delivering high quality person-centred care.

As a community and acute frailty geriatrician, you will also work closely with colleagues in acute and general medicine, and in the Emergency Department.

We are a provider of undergraduate medical school education for the University of Leeds and have a thriving research community.

We have been proud to develop our Frailty Education Faculty, running a range of frailty education sessions across the trust.

We have also recently appointed 2 dedicated Frailty Practice Educators who are in the process of rolling out EnCOP (Enhanced Care of Older People) competency training to the ARCH team.

The post-holder will also play a role in upskilling the existing team.

North Yorkshire has an ageing population, with 24.2% of the population in 2018 being over the age of 65.

Projections indicate that the population in the over 85 age group is expected to increase in Harrogate by approximately 30% by 2025.

HDFT provide both acute and community services, so in conjunction with our partners, we are in a great position to strengthen our frailty pathways from the front door of the hospital to our patients homes.
