    Description

    An exciting opportunity has arisen for a band 6 Urgent Care Practitioner to join the Ceredigion's 'Same Day Urgent Care service' (SDUC) team. The current advanced practitioner-led service provides alternatives to attending the main hospital sites for urgent care needs as well as accepting urgent primary care, providing a Minor Injury service and assessing, diagnosing and treating patients within their own homes via our community urgent care outreach service which also supports current community services within Ceredigion County.

    This post will require an enthusiastic, motivated and dynamic practitioner. Experience of practice in the community and urgent care arena is essential.

    The post holder will be expected to undertake advanced assessments and have the experience and training to develop care plans with the community/primary and acute MDT.

    The role includes night rotation, where direct referrals will be accepted from the out of hours GP's, or acute/community services, therefore skills and experience of IV drug administration, venepuncture, catheterisation, verification of expected death and setting up or managing syringe drivers is desirable although full training will be provided.

    Main duties of the job During the day rotations you will work within the SDUC team in CICC and as part of the outreach team covering the county, utilising triage skills, prescribing X-rays, and providing medications under PGDs or prescribed. The successful candidate will have relevant training and a scope of practice to work within, which will allow them to assess and treat patients with a variety of presenting complaints of both illness and injury under the supervision of senior urgent enhanced practitioners and advanced practitioners.

    The service is constantly developing, in line with changes in local and national healthcare provision and as such we are looking for a forward- thinking, competent, and caring practitioner with the skills and experience needed for triaging and assessing patients, and carrying out treatments, working closely with the ACP's and the community MDT.

    The successful candidate will be committed to service improvements with an acceptance of change, and desire to work in a positive environment that embraces the challenges urgent care faces as the service moves forward.The role will be to cover a 24hr service, 7 days a week.

    As part of the outreach team lone working will apply at times and the ability to travel between sites in a timely manner.     Seven community hospitals: Amman Valley and Llandovery in Carmarthenshire Tregaron, Aberaeron and Cardigan in Ceredigion and Tenby and South Pembrokeshire Hospital Health and Social Care Resource Centre in Pembrokeshire

    48 general practices (six of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres

    Person Specification Qualifications and Knowledge Essential
    • Registered practitioner (e.g NMC/HPC)
    • Postgraduate diploma level qualification or equivalent experience
    • Evidence of relevant in-house training or modules completed in Minor illness/Injury which covers patient assessment and clinical assessments at Level 6 or working towards
    • ILS/PLS Trauma course or evidence of in-house trauma training
    • Able to critically analyse research findings and apply to practice
    • Mainitain own personal and organisational development
    • Administers agreed drugs in accordance with locally agreed patient group directions (PGD's)
    • Completed Verification of Death and DNRCPR Training
    • Competent at phlebotomy / Cannualtion / IV drug admisintration
    • Triage trained
