    An exciting opportunity has arisen for practitioners to work within our lively and friendly community team based in the multi-cultural city of Bradford.

    We are looking for a skilled, enthusiastic, and passionate Band 6 Care Coordinator (CPN/Social Worker/Occupational Therapist) to join our fantastic Bradford South & West community mental health team.

    Covering the South & West area of our District, we enjoy working in one of the most diverse and culturally rich populations in England. We are always looking for new and innovative ways of delivering care to those with protected characteristics.     Main duties of the job As a Care Coordinator you are not only supporting service users through what can often be the most challenging of times, you are also mentoring them to become the best version of themselves. Successful applicants will be able to demonstrate a high level of commitment, motivation, and knowledge of working with people with severe and enduring mental health. You will need to demonstrate clinically effective care management skills and interventions and will have a good awareness of current health and social care issues which affect, or influence, our community.     • Professional Qualification (RMN, Social Work, Occupational Therapist) to degree level or equivalent
    • Evidence of continuing professional development.
    Desirable
    • Clinical supervision qualification.
    • Post Graduate level education in psychological therapy.
    EXPERIENCE Essential
    • Minimum of 6 months post basic experience in Mental Health
    • Mental health care/treatment relevant to service.
    • Working with people presenting in mental health crisis.
    • Multi-agency working/working across service interfaces.
    • Working within a multicultural framework.
    Desirable
    • Adult age related illness and conditions
    • Experience of teaching/leadership of others.
    KNOWLEDGE Essential
    • Evidenced based clinical interventions relevant to the service.
    • Relevant specialist clinical courses/training.
    • Models of care and treatment relevant to working with people in mental health crisis
    • Evidence of continuing professional development.
    Desirable
    • SystmOne PAS.
    SKILLS Essential
    • Assessing, formulating and working with people presenting in heightened state of distress.
    • Communicating (oral and written) complex/highly technical and clinically sensitive information to clients, their families, carers and other professional colleagues both within and outside the NHS.
    • Clinical risk assessment and contingency planning.
    • Able to meet the service needs for mobility across the geographical area covered

    • Brief psychological interventions/strategies for managing crisis.
