    Job summary

    Thepost holder will work as part of the Health Centre Team in collaboration withthe general practitioners and develop an effective, quality driven practicenurse service. The post holder will promote integrated nursing practice withinthe wider nursing team underpinned by the development of evidence-based practice guidelines, forshared delivery of care to the whole practice population. Further, to ensure clinical effectiveness bymonitoring of key quality markers relating to guidelines

    Main duties of the job

    Practiceindependently in accordance with NMC code of conduct and with reference topractice guidelines, appropriately referring to GPs.

    Toreceive and assess emergency patients and any incoming medical queries eitherby phone or face to face appointment and either treat or refer as appropriate.

    Managementof chronic diseases, in particular disease areas of Asthma and COPD anddressing suture removal, ear syringing, management of minor illnesses,vaccinations, immunisations, treatment of warts and verrucae either with orwithout liquid nitrogen etc following relevant training and in the absence ofnursing colleagues. any area as per procedure with training guidance.

    Toencourage health promotion and life-style counselling of patients asappropriate.

    Ensurecontinuation of own personal and professional development facilitated byutilisation of clinical supervision networks.

    Providegeneral and specific health screenings to the practice patients (within agreedprotocols) with referral to general practitioners as necessary.

    Key Responsibilities

    Clinical Practice

    Administration& Professional Responsibilities

    Ensureaccurate note of all consultations and treatments are recorded in the patientscomputer records, as required.

    Ensureaccurate completion of all necessary documentation associated with patienthealth care within the practice.

    To beinvolved in National Service Frameworks and Clinical Governance asrequired.

    Toparticipate in the production of new and revised protocols for the practice inconjunction with the GPs and Practice Manager.

    Attendand participate in practice meetings, including the Multi-Disciplinary Health Care Team Meetings as required.

    Tomaintain up-to-date personal professional development and awareness throughreading and attendance on courses as agreed with the lead GP.

    Tomaintain effective liaison with other agencies and staff concerned with patientcare and with all other disciplines within the practice with appropriate regardto confidentiality.

    Toensure that the key requirements of the current Health and Safety at Worklegislation are understood, in that it is the personal responsibility of everyemployee to work and behave in the manner which protects the safety of theircolleagues and the patients and to ensure their work place is maintained in asafe, clean and tidy condition at all times.

    Thepost holder will contribute to audit as requested by the Partners of the HealthCare Centre

    FamilyPlanning advice

    Diabetes,Asthma and COPD management

    Supportpractice to achieve QOF.

    Requestingappropriate pathology tests to support patient diagnosis.

    Act asa conduit to the patients registered GP for further advice where appropriate.

    Providenursing treatments to patients in participation with general practitioners orindependently to agreed protocols.

    Providegeneral and specific health screenings to the practice patients (within agreedprotocols) with referral to general practitioners as necessary.

    Advisepatients on general health care with referral to GPs as necessary.

    Communication andrelationships

    Toliaise and communicate with staff, managers, patients, and service users at alllevels

    Developeffective channels and systems of communication and ensure colleagues and staffare kept informed in a timely and appropriate manner

    Toensure that confidentiality is always maintained in conjunction with thePractice's Confidentiality Policy

    Play anactive role as members of interdisciplinary teams

    Understandthe roles of a range of professionals and carers from other organisations andsettings who may be participating in the care of a person and their family

    Person Specification

    Qualifications

    Essential

  • RGN - First Level Nurse/Adult Branch Specific Diploma or Degree
  • Desirable

  • Diploma or equivalent in Chronic disease Management (or working
  • Towards)
  • ENB 998 or equivalent
  • Experience

    Essential

  • 3 years post registration experience
  • Theoretical/practical knowledge of chronic disease management programmes and NSFs and willingness to work towards appropriate qualifications
  • Demonstrate the ability to give a high level of care to patients
  • Desirable

  • Evidence of a working knowledge in chronic disease management, health promotion, wound care, emergency management, Immunisation/ Vaccination and/or cytology
