Description
We require support workers for the Jean Heyes Reablement Unit based at Leigh Infirmary.

It supports individuals to achieve specific goals which enable safe discharge back into the community or their own home.

This post is a Monday to Sunday role with day and night shifts. Various hours available on both fixed term and permanent. Experience in a similar role is essential.

An Enhanced DBS with Adults' Barred List checks will be obtained during pre-employment checks.

This post involves contact with patients and families face to face within a community setting.

Therefore it cannot be ruled out that the postholder may come in to contact with people who have COVID-19, although the Trust provides PPE and takes all reasonable steps to minimise risk.

We are looking to recruit a highly motivated, enthusiastic staff to work as part of our multidisciplinary team.


The Jean Heyes Reablement Unit is run by a team of dedicated specialist community practitioners who are experts in providing the most appropriate care for our frail & elderly population.

Applicants will be required to see patients and families face to face within a community setting.

Responsibilities:


  • Post holders are required to work independently with regard to routine, delegated tasks, following established good practice and refer nonroutine tasks appropriately. Work is supervised and evaluated on a routine basis.
  • To ensure that documentation is completed in accordance with Trust and departmental requirements.
  • To be responsible for some degree of housekeeping and stock control on the Unit
  • To communicate with patients and/or carers to provide reassurance and promote understanding
  • Adhere to the Trusts Policies and Procedures
  • Communicate effectively in the receiving and providing of written, nonverbal and verbal information.
  • Maintain and adhere to relevant confidentiality, Data Protection Trust Policies applicable to role.
  • Ensure accurate records are kept.
  • Report complaints, accidents and untoward incidents according to Trust Policy.
  • Identify and report any risk management issues.
  • Observe rules governing health and safety as stipulated by the Trust Safety Policy.
  • Develop Information Technology (IT) skills in line with Trust Information and Technology Strategy.

  • To attend any inservice training programmes as appropriate
  • To take part in the individual appraisal and performance review programme and to actively participate in supervision.
  • To maintain own continuing professional development (CPD) by keeping up to date any developments and incorporate them as necessary into your work.
  • To respect the individuality, values, cultural and religious diversity of patients.
  • To exercise good time management, punctuality and consistent and reliable attendance.
  • To assist community assessment unit staff in the induction and training of new staff.
Planning and Organisational Duties

  • To be aware of Health and Safety aspects of your work and implement any policies, which may be required to improve the safety of your work area, including your prompt recording and reporting of accidents to senior stage and ensuring that equipment use if safe.
  • To maintain accurate, comprehensive and uptodate documentation, in line with legal and departmental requirements.
  • To comply with the organisational and departmental policies and procedures.
  • To undertake departmental administrative duties, as required.
  • To undertake any other duties that might be considered appropriate by the relevant registered nurse/Team Leader, or Community Therapy Services Lead.
Responsibility for Health & Safety


Compliance with the Health & Safety at Work Act the post holder is required to fulfil a proactive role towards the management of risk in all of their actions.

This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or
