    Defence Primary Healthcare (DPHC) has an exciting role available for a Band 6 Practice Nurse. We are a dedicated organisation working to provide excellent care to our hardworking personnel.

    We are looking for an organised, motivated and supportive Band 6 Practice Nurse to join our excellent team at Lossiemouth Medical Centre, RAF Lossiemouth.

    As the Band 6 Practice Nurse, you will work as part of a multidisciplinary primary healthcare team delivering primary healthcare nursing including elements of occupational health, working with the Senior Practice Nurse or Senior Medical Officer (SMO) within the medical facility (MF) and will be a focal point for Primary Healthcare nursing issues within area of practice.

    You will be able to work unsupervised and provide nursing care to the MF population at risk, holding an individual case load, and through consultation identify, assess, and diagnose patients with complex health needs, minor injuries, minor illnesses, and specific diseases within their level of competence.

    Have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporate the issues and recommendations made.

    Plan, manage and complete episodes of care, working in partnership with others and delegating and referring as appropriate to optimise health outcomes and resources use, as well as providing direct support to patients and clients.

    Interpret and analyse clinical and non-clinical facts to form accurate diagnosis, prognoses for a wide range of conditions, to recommend/deliver the best course of intervention including the development of comprehensive packages of care.

    Manage the care of patients with chronic illnesses e.g. Asthma, Diabetes, Chronic Heart Disease (CHD) and hypertension ensuring continued maintenance of the Chronic Disease Register.
    Undertake health and well-being clinics (e.g. smoking cessation, weight management, alcohol misuse) using health promotion, health education, screening, and therapeutic communication skills.
