The Role:
Joining our Older Adults Team as a qualified and experienced Social Worker, you'll be innovative, person-centred and committed to working in a strengths-based way. You'll use your excellent relationship management skills to collaborate and negotiate with individuals, their families, providers and other health care professionals, to proactively encourage independent and safe living within the community.

Travel Requirements:
You will be required to travel to various locations within Hampshire or other local authority, and it is essential that you have a current drivers' licence and access to a reliable vehicle on a regular basis.
