Help us to improve the health and wellbeing of thousands of people, and have a positive, long-term impact on their lives - and for generations to come.


We are redesigning the way that care and support for all of our population is delivered, and City and Hackney Neighbourhoods is at the heart of this change.

GP practices have joined with residents, local hospitals, community groups, mental health providers, social care, and voluntary sector organisations to create eight 'Neighbourhood' areas, each supporting populations of around 30-50,000 people, these are known as Primary Care Networks.

Focusing on what matters to local people, they are working together to take action and collectively address the population needs within each of these areas and increase co-ordinated multi-disciplinary support for residents.


In this newly-created role, you will play a critical part in helping us to achieve our ambitions, as you drive forward the transformation agenda of this £1m programme.


Part of your role will involve working with the Children, Young People and Families Team, planning and coordinating projects that bring together teams of practitioners to address specific needs across Neighbourhoods.

Working with the Central Neighbourhoods Team, you will coordinate the reporting of individually funded projects and support the delivery of cross-cutting projects across the Neighbourhoods programme.

You will also be involved in communications for the overall programme.


This is a varied role, offering opportunities to gain new experience and valuable transferable skills as you help to develop new approaches and strengthen ways of working.

Developing and maintaining relationships with a broad range of partners and senior stakeholders, you will share and grow your experience learning from colleagues working across this system-wide programme.

We would like to hear from you if you have experience of monitoring project delivery and co-ordinating project reporting.

We would also like you to bring knowledge of health and social care, and an understanding of policy, systems and practice in Local Authorities, the NHS and Voluntary and Community sectors.


With experience of supporting change initiatives and projects that involve multi-agency working across organisations, you will have the ability to engage and communicate with a broad range of stakeholders including residents, young people and colleagues at all levels, using a range of different channels.

You should also be able to prioritise and organise delegated workloads, and work in a flexible environment, individually and as part of a team.
